Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards and crush hazards while being lifted by the forks of the rough terrain truck: Worksite- The two employees being lifted by the forks of the Shaughnessy rough terrain forklift truck were not protected by a properly designed personnel platform equipped with guardrails. Among other methods, one feasible and acceptable method to correct the hazard is to: 1) Follow the requirements of the American National Standard Institute standard, ANSI B56.6-1992, Safety Standard for Rough Terrain Forklift Trucks. 2) Follow the manufacturer's operating procedures.
