Worksite- The two employees being lifted by the forks of the Shaughnessy
rough terrain forklift
truck were not protected by a properly designed personnel platform
equipped with guardrails.
Among other methods, one feasible and acceptable method to correct the
hazard is to: 1) Follow
the requirements of the American National Standard Institute standard,
ANSI B56.6-1992, Safety
Standard for Rough Terrain Forklift Trucks. 2) Follow the manufacturer's
operating procedures.
