The employer failed to ensure that a truck back-up alarm worked properly:
a.At the transferring/loading area, an employee was fatally injured when
he was
stuck and pinned in between a truck and a loader rail. The Kenworth Semi
Truck
#42, license plate 26254A was operating in reverse with no operating
back-up
alarm.
One feasible and acceptable abatement method to correct this hazard would
be to
ensure drivers check to see the back-up alarm is operating before moving
the
truck.
