Nevada Revised Statute 618.375(1): The employer failed to furnish employment and
a place of employment which was free from recognized hazards that were causing
or were likely to cause death or serious physical harm to his employees: The emp
loyer failed to ensure that an appropriate personnel platform was used to elevat
e an employee to work on the top of a wall that was 13' above the ground, as fol
lows: a.On the south side of the building located at 5920 South Rainbow Boulevar
d in Las Vegas, NV 89134, an employee was standing on top of unsecured 4x8 sheet
s of plywood that had been placed across the forks of the rough terrain forklift
(ID 335997). Reference: ASME B56.6B - 1998, American Standards for rough Terrai
n Forklifts - paragraph 5.15.1. A rough terrain forklift shall not be used to li
ft people
unless there is no other practical option. One feasible and acceptable abatement
method, among others would be to follow the procedures outlined in ASME B56.6B
- 1998, paragraph 5.15.raph
