Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: the hazards of severe injuries and or death associated with exposure to fall hazards of 9-18 feet high. a) Employee(s) were observed performing steel working operations while elevated on a work platform suspended on the forks of a Model 534D10-45 Gradall Rough Terrain Forklift. The following hazards were observed: 1. Employees were observed working from a twenty foot (240 inches) long work platform and this dimension exceeded the maximum allowable width of 118.8 inches for this Gradall machine (width of Gradall Model 534D10-45 is 98.8 inches). 2. The work platform did not have the maximum work load and empty weight information prominently marked on the platform. One feasible method of correcting these hazards is to implement and follow ANSI/ASME B56.6-1992 Section 8.25; Platforms for Elevating Personnel: 8.25.1(b): Use a work platform that is no longer than the width of the machine plus 10 inches on each side of the machine. 8.25.1(f)(1): Ensure that the work platform used has the maximum work load prominently indicated on the platform. 8.28.1(f)(2): Ensure that the work platform used has the weight of the empty platform prominently displayed on the platform.
