Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of: a) For the period of time up to and including 08 Jul 04, the employer did not always ensure that the cable of the air hoist located on the drill floor (away from the driller) was in good condition and properly wound on the hoist's drum. The cable of the air hoist away from the driller was kinked and permanently deformed and was also not wound properly on the drum. This condition exposed employees to the hazards associated with the cable breaking and/or the cable becoming entangled on the drum. American Petroleum Institute (API) Recommended Practice RP54 Paragraph 6.3.2 states that: "Well Operations involving the rig shall not be commenced until the rig is rigged up in a safe manner." API Recommended Practice RP9G Paragraph 3.2.7 states that: "Care should be taken to avoid kinking a wire rope since a kink can be cause for removal of the wire rope or damaged section." API Recommended Practice RP9G Paragraph 3.3.3 states that: "Rope should be kept tightly and evenly wound on the drums." Among other methods, one feasible and acceptable abatement method to correct this hazard would be to: Perform daily inspections of wire rope used on site and remove from service any wire rope that is damaged. During inspections the hoist cable guide should be inspected to ensure that the cable is wound on the drum properly.
