preventer.
a)  Among other methods, one feasible and acceptable method to correct
this hazard
would be to follow recognized industry practice by ensuring that a minimum
number of
bolt assemblies remain secured in place on the blow out preventer flange
prior to the
crane hook being placed in the lifting bale.
