recognized hazards that were causing or likely to cause death or serious
physical harm to
employees in that employees were exposed to: being crushed by unsecured 5'
x 10' slabs of
quartz weighing 706 pounds each.
a)On or about 01/17/2006 - warehouse and yard, 36-16 19th Avenue
Employees stand in the "fall zone" while handling unsecured 5' x 10'
quartz slabs, stored
horizontally on A-frame racks in order to attach a lifting clamp to the
center of the slab.
Up to 30 slabs are stored on each side of the A-frame racks and each slab
weighs 706
pounds.
AMONG OTHER METHODS, FEASIBLE AND ACCEPTABLE METHODS TO
CORRECT THIS HAZARD WOULD BE TO:
1. Install restraining devices to A-frame such as securing arms which can
be lowered and angled
to secure multiple slabs while in storage and to prevent slabs from
falling over when being
handled by employees.  Restraining devices must be able to support the
maximum weight
capacity of the storage unit.
2. Develop and implement safe slab handling procedures, specifically
addressing separting slabs
and attaching lifting devices, when loading and unloading A-Frame storage
systems to prevent
employee exposure to falling slabs.
