Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to fall hazards:
A) On 4/13/06: Terminal Four: Virgin Atlantic Flight #3
Stinar Cleaning truck #8572 was not positioned such that the bumper of the
extendable platform
was in contact with the aircraft. Employees were exposed to falling
through angled unguarded
opening, having a maximum distance of 18" wide, between the bumper of the
extendable
platform and the aircraft rear entrance door.  The platform was raised
approximately 16' above
the ground.
B) On 4/14/06: Terminal Four: Virgin Atlantic Flight #3
Stinar Cleaning truck #8572 was not positioned such that the bumper of the
extendable
platform
was in contact with the aircraft. Employees were  exposed to falling
through unguarded opening
that varied from 14 1/2" to 9" wide, between the bumper of the extendable
platform and the
aircraft rear entrance door.  The platform was raised approximately 16'
above the ground.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS TO:
Prior to employee usage, ensure that high lift cleaning trucks are
positioned such that the
platform bumper is in contact with the aircraft when the platform has been
raised and extended.

front section of
platform using the Ramp Out/In toggle switch until platform bumper
contacts
aircraft.
2) American National Standard for Airline Ground Support Vehicle-Mounted
Vertical Lift
Devices A92.7-1990): Section 14.2.1: During Operation which states the
unit shall be used only
in accordance with the manufacturer's limitations and the owner/operator
instructions and safety
rules.
