North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to: a) facility wide -- when asked, the employer failed to provide any record of periodic inspections of the crane in service at the time of the inspection ( 2 Ton jib crane in mechanical shop, work bays G through I ). REFERENCE: ANSI B30.11-2.1.5: Dated evidence of the inspection of critical items such as hoisting machinery, sheaves, hooks, chains, ropes, wheels, and other lifting devices, as required in 11-2.1.1(b)(2), shall be maintained. This evidence should be kept where available to appointed personnel.

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to: a) facility wide -- when asked, the employer failed to provide record of load test of the crane in service at the time of the inspection ( 2 Ton jib crane in mechanical shop, work bays G through I ). REFERENCE: ANSI B30.11-2.2.2: Prior to initial use, all new, extensible repaired, and altered equipment should be tested and inspected by, or under the direction of, an appointed or authorized person, and a written report should be furnished by such person, confirming the load rating of the system. The test reports shall be place on file.

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to: a)facility wide -- it was observed that the below the hook lifting devices in service at the time of the inspection ( metal thong ), did not have any markings for the rated load. b)facility wide -- it was observed that the below the hook lifting devices in service at the time of the inspection ( metal thong ), did not have a nameplate or other permanent markings for the identification of manufacturer name or serial number. REFERENCE: ANSI B30.20-1.2.1( 1968 ), (a )Rated Load. The rated load of the lifting device shall be marked on the main structure where it is visible.(b)Identification. A nameplate or other permanent marking shall be affixed displaying the following information: (1) manufacturers name and address, (2) serial umber, (3) lifter weight (4) rated load.

REFERENCE: ANSI B30.20-1.3.6( 1968 ), Dated inspection records shall be made on critical items such as those listed in para. 20.1.3.4. Records should be available to appointed personnel for each periodic inspection and when the lifter is either modified or repaired.
