which were free from recognized hazards that were causing or likely to
cause death or serious
physical harm to employees in that employees were exposed to:
In that the seat belt of the Toyota Electric powered industrial truck was
not being utilized as
required to prevent the operator from being crushed by the overhead guard
in the event of a
tipover.
Among other methods, one feasible and acceptable method to correct the
hazard associated with
this item would be to provide additional training to employees, and to
enforce a disciplinary
policy that would ensure that all operators of the powered industrial
truck always wear their seat
belt.
