were exposed to THE HAZARD OF BEING STRUCK-BY THE LOAD:
a)Paint spray area overhead hoists, on or about 07/09/2004: Two one ton
overhead electric hoists, Serial Numbers unknown had no frequent or
periodic inspections conducted nor records maintained of those inspections.
b)Prefab area overhead motor hoist, on or about 07/09/2004: One 3 ton Twin
City Overhead Electric hoist, serial number unknown had no frequent or
periodic inspections conducted nor records maintained of those inspections.
c)Band Saw area Overhead hoist, on or about 07/09/2004: One 3 ton Twin City
electric overhead hoist, serial number unknown had no frequent or periodic
inspections conducted nor records maintained of those inspections.
d)Fab shop area near overhead shipping/receiving door, on or about
07/09/2004: One 5 ton electric overhead hoist, unknown name and serial
