recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to A FALL HAZARD:
a)In the warehouse, on or about 10/13/04:  Employees were operating a high-
lift order picker, #021, using a retractable lanyard and body belt.  The
retractable lanyard was attached to the overhead protection system of the
order picker by a non-locking carabiner.  Employees were exposed to a fall
hazard of approximately fifteen feet if the order picker was fully
extended at
the time of the fall.
Abatement certification must be submitted for this item.
