Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard created by the sudden and unpredictable descent of a vehicle elevated by an automotive lift: a)In the Service Area, on or about 12/17/04: Ammco, two post automotive lift, model number AL-7, serial number P000006651 was missing the locking device for the left front swing arm. b)Anchor bolts on the Ammco two post lift, serial number P000006651, were not re-torqued to one hundred fifty foot pounds on an monthly basis as required by the lift manufacturer. c)The hydraulic pump on the Ammco two post lift, serial number P000006651, was leaking around the top seal. d)The employer did not train and document employees were instructed in the safe use and operation of automotive lifts. Among other methods, several feasible and acceptable abatement methods to correct these hazards are to: 1.Install locking device for the front swing arm. 2.Establish a periodic inspection as recommended by the manufacturer, to include re-torquing of the anchor bolts and repairing all leaks, to ensure the safe operation of the lift. 3.Document that all employees have received training in the safe use and operation of automotive lifts. Abatement certification must be submitted for this items.ted
