Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY THE LOAD: a)Maintenance Shop, on or about 01/13/05: A vehicle hoist being used for maintenance had not been inspected. b)Maintenance Shop, on or about 01/13/05: A vehicle hoist being used for maintenance was not labeled for maximum load capacity. c)Maintenance Shop, on or about 01/13/05: A vehicle hoist being used for maintenance had an inoperative safety lock. d)Maintenance Shop, on or about 01/13/05: A vehicle hoist being used for maintenance was missing 3 safety pins for the lifting plates. ABATEMENT NOTES: Among other methods one feasible and acceptable method to correct this hazard is to: 1.Conduct periodic inspections of the vehicle hoist as per ANSI/ALI ALOIM-2000. 2.Label the hoist with the maximum rated load capacity. 3.Repair the safety lock on the vertical lifting device. 4. Replace the missing safety pins on the lifting plates. Abatement certification must be submitted for this item.
