a)West side of building, on or about 3/9/05:  An employee operating a
Gradall rough terrain forklift was exposed to a crushing injury from being
caught between the rollover protection (ROPS) and the ground because the
installed seat belt was not being worn.
Among other methods, one feasible method to reduce this hazard is to
enforce the wearing of seat belts.
