Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of amputation from high pressure water
operating at 38,000 pounds
per square inch (PSI):
a. On or about 07/28/2005, in the strip and wash shop: Employees operate
high pressure Pratt& Whitney/NLB waterjetting equipment at a working
pressure of 38,000 PSI. The Waterjetting equipment was defective in that
the
safety trigger switch was not operational leading to the hazard of
amputation
if the gun was dropped because the switch would not disengage and the
38,000 PSI stream could hit the operator.
b. On or about 07/28/2005, in the strip and wash shop: Employees were not
trained to inspect the equipment for defects.
Among others some feasible abatement methods are to follow the safety
procedures in the manufacture's operations manual, including but not
limited
to the following:
1.(Sec. 5.5) Require the daily inspection of the equipment and replacement
of worn or defective equipment.
1.(Sec. 10.2) Provide operator training about high pressure waterjetting
equipment.
Abatement certification must be submitted for this Item.
