employee was exposed to a crushing and /or struck by hazard:
a).  Taylor Road Water Main Project.  On or about 5/19/04, an employee was
observed
riding on the bucket of a Komatsu WA300 front-end loader traveling east on
Taylor Road
and exposed to falling from the bucket and being crushed and/or struck by
the loader.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS TO FOLLOW THE MANUFACTURER'S OPERATION
& MAINTENANCE MANUAL WHICH PROHIBITS ANYONE FROM RIDING ON
THE BUCKET OR ATTACHMENTS.
