employees to a fall hazard resulting in broken bones and head injuries,

warehouse area was
maintained and/or furnished with a restraint system for employees uses as
required by
ASME/ANSI B56.1-1988, Section 7.34.1.3.  Feasible and acceptable abatement
methods
include but are not limited to the following:
Employer must follow the instruction of the operator manual which states
"When using
an order picker lift truck, the operator shall wear the safety restraint
device at all times
during operationof the unit".
Employer must replace and/or repair the defective part (restraint system)
before operating
the "Raymond" order picker.
