Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to injuries from a horizontal boring machine.
On or about 06/21/05, at State Hwy 42 and South Singley Dr., Locast Grove,
GA:
a) Employee was exposed to an amputation injury hazard. Employee was
injuried while walking
by an operating (in reverse) horizontal boring machine with the spoil
chamber door open.
"AMONG OTHER METHODS, ONE FEASIBLE, AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS":
1) Instruct each operator and trainee during initial training and at
safety meetings, to shutdown
the horizontal boring machine whenever installing and pinning an auger.
2) Instruct each operator and trainee during initial training and at
safety
meetings, to close the
spoil chamber door whenever the horizontal boring machine is operating.
3) Do not modify the horizontal boring machine, ie. Control panel: removal
of electrical circuit
breakers, light switch, engine hour operating indicator and the
replacement of the red E-STOP
(PUSH-TO-STOP control) with a toggle switch.
4. Conduct periodic monitoring inspections of horizontal boring operations
and correct
deficiencies immediately.
