Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed fall and struck-by hazards: On or about 06/30/05, at the PCS Nitrogen Fertilizer, L.P., Augusta, GA: a) Employees were exposed to a fall and struck-by hazard. Employees were working from a platform on a JCB Loadall rough terrain forklift elevated greater than 20 feet above the ground, while the platform was tilted forward. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS": Follow the guidance set forth in ASME B56.6-1992 - Safety Standards for Rough Terrain Forklift Trucks; 5.15 Elevating Personnel; 5.15.1(i) Be certain that the platform is horizontal and never tilted forward or rearward when elevated.
