were exposed to:
On or about 01/05/06, at Brickton North, Lot 12, Buford, GA:
a) Employees were exposed to a fall hazard. Employees were working in an
approved work
platform attached to a SkyTrak 6036 rough terrain forklift at
approximately 10'-12' above a
concrete surface without wearing a safety harness with lanyard.
b) The Operator of the SkyTrak 6036 rough terrain forklift
moved/repositioned the lift while
employees were on the elevated platform.
c) The operator got out of the lift cab and left the lift controls
unattended
while employees were
in the elevated work platform.
" AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS"
Follow the guidance set forth in ASME B56.6-1992; Safety Standards for
Rough Terrain Forklift
Trucks; 5.15 Elevating Personnel; 5.15.1(k), (q) and (s). Be certain that
required restraining
means such as railings, chains, cable, body belt(s) with lanyard(s), etc.,
are in place and
properly used, lower platform for adjustments in positioning and operator
in position to control
forklift truck.
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
