recognized hazards that
were causing or likely to cause death or serious physical harm to

were exposed to the hazard of being struck by an object which could cause
serious injury or
death:

A) Greece Arcadia High School, rear of the building. Employees were
exposed to injury
resulting from the bucket of a Case model 9030B Excavator not being
lowered to the
ground prior to parking the vehicle, as articulated in the operator's
manual, and the
company safety manual.
Among other methods one feasible and acceptable method to reduce or
eliminate this
hazard is to establish and enforce safe operating procedures such as those
noted in the
Case model 9030B Excavator Operator's Manual. Elements essential to a safe
operating
procedure include at a minimum (1) Development of a directive which would
prohibit
operators from parking and/or exiting the excavator without lowering the
bucket safely
to the ground, and (2) Provide education and training to all operators of
the safe
operation of this type of equipment.
