were exposed to fall hazards while being lifted on an elevated platform:
(a) Springhill Plaza Site: Employees were exposed to fall hazards when
lifted to the third
story, approximately 30 feet, with a Rough Terrain Forklift Truck using a
platform that
extended more than 10 inches beyond the width of the truck on both sides.
One feasible method to correct this hazard is to follow the requirements
specified in
ANSI Standard B56.6-1992, "Safety Standard for Rough Terrain Forklift
Trucks,"
Paragraph 8.25.1(b), "Platforms for Elevating Personnel" which states,
"Floor
dimensions shall not exceed two times the load center distance listed on
the rough terrain
forklift truck nameplate, measured parallel to the longitudinal center
plane
