Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to fall hazards while being lifted
on an elevated platform:
a)North side of building: Employees were exposed to fall hazards when
lifted approximately 23 feet to
the roof on a 286 inch long platform via a GEHL, Model RS5, all terrain
forklift.  The platform
extended approximately 88 inches beyond each side of the truck's wheel
base (measured across the
load bearing tires).
One feasible method to correct this hazard would be to follow the
requirements specified in ANSI
Standard B56.6-1992, Safety Standard for Rough Terrain Forklift Trucks,
Paragraph 8.25.1(b),
Platforms for Elevating Personnel, which states: "Floor dimensions shall
not exceed two times the load
center distance listed on the rough terrain forklift truck nameplate,
measured parallel to the
longitudinal center plane of the truck, nor have a width greater than the
overall width of the truck
(measured across the load bearing tires) plus 10 inches on either side.
