North Carolina General Statute 95-129(1) of the Occupational Safety and
Health Act of North Carolina:  The
employer did not furnish to each of his employees conditions of employment
and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to
employees, in that employees were exposed to a struck-by falling objects
hazard:
a)jobsite:  on or about 12:20 hours on May 7, 2004 an employee was
operating a crane in
preparation for a lift and two-blocked the auxiliary line ball and hook
with the auxiliary
sheave, causing the ball and hook to fall, exposing employees to a
struck-by falling objects
hazard.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to ensure that an anti-two block system is installed and operating on all
lines that are reeved
for action.

crane without being under
the direct supervision of a designated operator and two-blocked the
auxiliary hoist line, causing
the ball and hook to fall, exposing employees to a struck-by falling
objects hazard.
