Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential injuries from fall hazards. a) Northwest exterior wall where fish are placed into truck transport tanks: Workers walking on a 20 inch wide aluminum plank used to walk between a 7 foot high wall and a parked fish transport truck were exposed to potential injuries from falls (concussion,broken bones, etc.) due to the lack of a guardrail or other fall arrest system, to prevent employees from falling to the ground, on or about 09/22/04. b) Fish plant/twelve fish tank raceways: Workers walking on 12 inch wide walkways separating 12 fish holding tanks were exposed to potential injuries from falls (concussion, broken bones, etc.) due to the lack of a guardrail or other fall arrest system, to prevent employees from falling into fish holding tanks, on or about 09/22/04. a) Among other methods, one feasible and acceptable abatement method to correct this hazard is to install guardrails and midrails onto the aluminum plank to prevent falls. b) Among other methods, one feasible and acceptable abatement method to correct this hazard is to install guardrails and midrails onto the walkways or to install an overhead static line system to which employees could attach a personal fall arrest system. ABATEMENT CERTIFICATION REQUIRED

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to injuries from possible contact with hydrochloric acid while applying the acid to drum micro screens and while dispensing it from two 55 gallon containers into spray containers: a) Fish farm building: Workers dispensing hydrochloric acid from two 55 gallon drums and who spray the acid on the drum micro screens were not furnished with facilities for quick drenching and flushing of the eyes and body, exposing them to possible chemical burn injuries, on or about 09/22/04. Among other methods, one feasible and acceptable abatement method to correct this hazard is to provide plumbed emergency eyewash units in an area close to acid dispensing operations. (See ANSI Z358.1-2004). ABATEMENT CERTIFICATION REQUIRED
