that were causing or likely to cause death or serious physical harm to
employees in that an
employee was exposed to multiple injuries from falling off a back hoe,
when no seating was
available.
a) Twin Hills Memorial Park, 3332 Lycoming Mall Drive, Muncy, PA:  A back
hoe
operator allowed an employee to ride on the side of the equipment with no
safe seating
available, on or about January 18, 2005.
Among other methods, one useful and acceptable method to correct this
hazard is to establish
and enforce a safety rule prohibiting employees from riding on the back
hoe in positions that
are not provided with safe seating accommodations and seat belts.
