recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to crushing and/or struck by hazards when the
vision of the
Shuttlelift operator was obstructed during tramming operations.
a.)Outside Trailer Loading Area - Employees had to walk with the gantry
crane during
tramming operation but the operator was unable to see them when they were
positioned
behind it, on or about October 29, 2005.
One feasible and useful method to correct this hazard is to adhere to the
Safety Rules
contained in the manufacturer's Operation and Maintenance Manual, ISL 30,
pages 1 through
10.
Specifically, and as an example:
A copy of the Safety Rules must be posted in a prominent place in the
office or on the job
site. All operating personnel must be aware of, and practice, these rules.
SAFETY RULES
Warning: Always have a signalman on the ground to guide you when traveling
with a load on the machine. Ensure that the signalman is in full view at
all times.
DO NOT allow unauthorized persons near machine when traveling or hoisting.
Agree
in advance on the set of signals to be used. (See page 1-8).
Warning:Do not attempt to travel with machine until signalman gives a
signal
that machine is clear of all obstacles in the wheel path of the machine.
Remove wood
blocks and blocking from the wheel path before traveling.
