Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by, caught between, and crushing hazards: On or about May 4, 2006 a Caterpillar 920 front-end loader was operated with limited visibility due to a cracked front windshield, a crack rear window, and no rear-view mirror. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED Among other methods, one feasible and acceptable method to correct this hazard is: a) Replace both windows in the Caterpillar 920; b) Install a manufacturer approved rear-view mirror; c) Restrict foot traffic in areas where this machine is operated; d) Instruct operators and others in the recognition of hazards associated with the operation of mechanical handling equipment.
