29 CFR 6a: Section 6(a) of the Puerto Rico Occupational Safety and Health
Act: The employer
did not furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or physical harm to employees
in that employees were
exposed to:

patrono no proveyo empleo y un sitio de empleo libre de riesgos
reconocidos que esten causando
o que pueda causar muerte o dano fisico a los empleados, por cuanto los
empleados estaban
expuestos a:
At the switchyard's substation in the Centro de Servicios de Emergencia de
Bayamon',
the employees were working in the maintenance of the  OCB 0032T and other
employees
were working in conservation putting gravel on the soil in the switchyard
when the
breaker OCB 4340 trade mark Westin House No. 33-252-I-A exploted. The
employeer
did not give inspection and maintenance to the breaker OC 4340 as the
manufacturer
instructions, that each breaker be given a general inspection established
in a planned
program. In this case the inspection was supposed to be furnished every
four years and
the last inspection was conducted on October 16, 2000.
Hazard: Explotion.
Among other methods, one feasible and acceptable abatements method to
correct
this
hazard is, replace the OCB-4340 and development a planned program for
inspection the
breakers.
En el patio de interruptores del Centro de Servicios de Emergencia de
Bayamon, los
empleados que estaban trabajando en el mantenimiento del OCB 0032T y otros
que
trabajaban en conservacion depositando gravilla en el terreno del patio de
interruptores,
cuando el interruptor OCB-4340 marca "Westing House"  No. 33-252-1A
exploto. El
patrono no dio inspeccion y mantenimiento del interruptor OC-4340 como se
recomendaba en las instrucciones del manufacturero para cada interruptor,
la cual era
hacer una inspeccion general establecida en un programa planeado.  En este
caso la
inspeccion estaba supuesta hacerse cada cuatro anos y la ultima inspeccion
conducida
fue para 16 de octubre de 2000.
Riesgo: Explosion.
Entre otro metodos, un metodo posible y aceptable para corregir este
riesgo es,
remplazar el interruptor OC 4340 y desarrollar un programa planeado de
inspeccion a
los interruptores.
