Two employees were observed working from a rough-terrain forklift
platform/basket,
approximately 20' above grade, without the use of a personal fall arrest
system or a fall
restraint system.  Additionally, guardrails were not installed around the
platform/basket,
the platform/basket was not secured to the forks or mast, and the
platform/basket
extended greater than 10" past the outside of the wheelbase.
