North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)at the site, employees were working 14'9" above lower levels without the use of a fall protection measure. Reference:Southern Chapter International Society of Arboriculture, First Edition, 2002, page 26. Note:Among other methods, one feasible and acceptable method to abate this hazard is to provide a fall protection measure. b)at the site, employees were using an extension ladder for access to an upper landing surface without the ladder side rails extended at least 3' above the upper landing surface. Note:Among other methods, one feasible and acceptable method to abate this hazard is to extend the side rails at least 3' above the landing surface.

NOTE:Among other methods, one feasible and acceptable method to abate these hazards is to provide ear, eye, face, head, and leg protection.

Reference:Southern Chapter International Society of Arboriculture, First Edition, 2002, page 35 & 54. NOTE:Among other methods, one feasible and acceptable method to abate this hazard is to provide a first-aid kit in accordance with the above listed reference. b)at the site, employees were not trained in first-aid and cardiopulmonary resuscitation techniques. Reference:Southern Chapter International Society of Arboriculture, First Edition, 2002, page 34 & 54. NOTE:Among other methods, one feasible and acceptable method to abate this hazard is to provide the required training.
