Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed severe lacerations:
(a) Outside Lab Area:  On or about February 8, 2005, an emergency stop
button was not located
at the operator's station on the Pendraulik mixer.
(b) Outside Lab Area:  On or about February 8, 2005, the on/off switch for
the Pendraulik
mixer was not so located that the operator could cut off power to the
machine without leaving
his/her position at the point of operation.
(c) Outside Lab Area:  On or about February 8, 2005, employees were not
trained to safely
operate the Pendraulik mixer in accordance with the manufacturer's
instructions.
Among other methods one feasible and acceptable means to correct the
hazard is to install an
emergency stop push button and the on/off switch to this mixer that is
within the reach of the
operator while stationed at the point of operation.  Also, to train the
employees in the safe
operation of the mixer in accordance with the machine's owners/operators
manual.
