3-12-05 an employee
sustained a fatal head injury when the employee fell onto the concrete
floor from a chair with
wheels that the employee was using to stand on to gain access to parts on
a top shelf.  No ladder
was provided for employees to use to access parts in the storage area.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to provide a ladder or step-stool for employees to use when retrieving
parts from the shelving
units in the storage area.
