installed under supervision of qualified engineer knowledgeable of hazards. 3.No Separation/segregation/detachment of combustible dust handling areas occurred within the facility. 4.The interior surfaces were not designed to minimize dust accumulations. 5.No written management of change procedures were established to manage change to process materials, technology, equipment, procedures and facilities. 6.Continuous suction was not installed to minimize the escape of dust. 7.Operating and maintenance procedures and emergency plan were not developed & reviewed annually including initial and refresher training. 8.An inspection, testing & maintenance program was not developed and implemented, with records kept of maintenance and repairs performed.
