Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a crushing hazard, in that:
a) On or about 9/27/04, a 1998 GM 16 foot box dual wheel tool truck
weighing about
16,017 pounds with tool was lifted on a 12 ton bottle jack with both rear
tires on the
passenger side removed. Wheel chocks were not in place under the front
wheel to
prevent the truck from rolling. The manufacturer of the truck requires the
use of jack
stands and chocking of the wheels at the end of the vehicle opposite the
end being lifted.
A feasible and acceptable method, among others, to abate this hazard is
(1) to follow
manufacture's guidelines: (2) Place Chocks on wheels which are not raised
off the
ground per manufacturer's requirement. (3) Ensure employees are familiar
with
the
equipment and the manufacture's instructions on its operation; maintenance
in accordance
with ANSI standard titled "JACKS" B30.1-1975 section 1-3-2.(a) "Recommended
Operation Procedures."
