a) On or about 10/28/04, in the brake shoe riveting area; the foot pedals
of the National
Rivet Machine Model 800 (SN 651H1) and the two Chicago Rivet Machines
Model 91
( no serial #), were not protected to prevent accidental actuation by
falling or moving
objects, or by inadvertent actvation by foot traffic.
Among other methods, one feasible and acceptable method of abatement is to
install a
guard that protects the foot petal from accidental or unwanted contact.
