Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: On or about April 12, 2005; the self-retracting lanyard was not installed on Rig #3 by a qualified engineer in accordance with the manufacturer's operators manual. Feasible methods to correct the hazard include, but not limited to, installing the lanyard under the supervision of a qualified engineer in accordance with paragraph 1, page 17 of Section 5.5 of the operator's manual.r's
