Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: On or about December 30, 2005, in the yard area; the total weight of the load below the hoist hooks was on the MI-JACK, model 750D, gantry crane was not known by the crane operator. Among other methods, one feasible and acceptable abatement method to assure the gross weight of the load is known by the crane operator in accordance with the operator's manual and the ASME Standard B30.20, Section 20-1.4, Below-the-Hook Lifting Devices.

Among other methods, one feasible and acceptable abatement method to correct this hazard is to affix data plates to the spreader bar in accordance with ASME Standard B30.20, Section 20- 1.2.ion

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: On or about January 13, 2006, in the yard area; no frequent or periodic inspection program for the MI-JACK, models 500, 650D and 750D, gantry crane, including but not limited to: a)structural members for deformation, cracks or excessive wear on parts of the lifter b)loose or missing guards, fasteners, covers, stops or name plates c)all functional operating mechanisms and automatic hold-and-release mechanisms for misadjustments interfering with operations d)loose bolts e)cracked or worn gears, pulleys, sheaves, sprockets, bearings, chains or belts f)excessive wear of friction pads, linkages and other mechanical parts g)excessive wear at hoist hooking points and load support devises or pins Among other methods, one feasible and acceptable abatement method would be to conduct inspections in accordance with ASME Standard B30.20, Section 20-1.3.
