Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to not having adequate training per American National
Standard Safety
Requirements for the Construction, Care, and Use of Die Casting Machines
ANSI B152.1-1973
7.2 Instruction of Operators. The employer did not train and instruct the
operator in the safe
method of work before starting work on any operation.  The employer did
not ensure that the
correct operating procedures were being followed:
1723 W. Industrial Drive, Jefferson City, MO.:
1)Employees did not have adequate training as employees were unjamming
stuck
shot tips by themselves, instead of seeking help.  Employees were
operating die
cast machines with rubber bands around the limit switch so that the die
cast
machines could be operated with the door open.
2)An employee injured in an accident involving Die Casting Machine #1 did
not know where the manual, and automatic switch were located on the Die
Casting
Machine and the difference it made to the operation of the machine.
While ultimate responsibility for correcting the hazard rest with the
employer,
given his/her superior knowledge of the workplace, feasible and acceptable
methods to correct this hazard, among others, include:
1)Ensure that employees have adequate training, and that they know they
are not
to unjam a stuck shot tip alone in any situation.  Document training given
to
employees covering the die cast machines.  Train employees that the use of
rubber bands or any method to bypass the limit switch is strictly
forbidden. Have
an enforcement program regarding not following safety procedures.  Remove
all
rubber bands when gloves enter the plant so they are not available to
personnel.
Remove or modify piece of metal that the rubber band was being attached to
on
the machine so that it can not be used to bypass limit switch.
On or about January 6, 2006, an employee received an amputation injury
from the die
casting machine cycling and not stopping when the employee was in the
process of
unjamming the stuck shot tip.
Abatement certification is required for this violation (using the
CERTIFICATION OF
CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating
that
abatement is complete must be included with your certification.  This
documentation may
include, but is not limited to, evidence of the purchase or repair of the
equipment, photographic
or video evidence of abatement, or other written records.
