Carolina:  The employer did not furnish employment and a place of
employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to:
a)jobsite, two employees were exposed to fall hazards while being
transported on
a personnel platform attached to the forks of a  LULL 1044C-54 Lift Truck;
the
personnel platform was missing the front guardrail.
Among other methods, one feasible and acceptable method of abatement to
correct this hazard is to provide a personnel platform which complies to
the
design requirements listed in ANSI/ASME B56.6-1992, Safety Standard for
Rough Terrain Forklift Trucks, and to secure the platform to the lifting
carriage
or forks.ge
