Section 5 (a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment which were free from recognized hazards that were
causing or likely to
cause death or serious physical harm to employees, in that, employees were
cleaning energized
machinery exposing them to amputation hazards from rotating parts.
North Street, Wrentham - Compost 2:
On 5/13/05 an employee suffered a partial amputation of his right arm
while cleaning the
rotating feed hopper on the Finn Bark Blower which was inadvertently
activated by a
remote control unit.
Methods to correct the condition, among other methods, are as follows:
1)  Establish a lockout/tagout program and train the employees on the
requirements of
the program prior to cleaning equipment.
2)  Repair the micro-switches to ensure the guards are affixed and secure
on the machine
at all times.
