were exposed to fall hazards while utilizing a rope descent system
supported from an inadequate
anchorage point:
Location:  175 Cambridge Street, Boston, MA
On 10/24/05, the lifeline was not rigged independently from the
transportable counterweighted
outrigger used for the primary suspension line exposing the employee to
fall hazards of up to
5 stories.  The lifeline and the primary line were tied together in a knot
attaching them both to
the outrigger.  Also, the outrigger was not tied-back to a certified
anchorage on the building.
A feasible abatement method includes, but is not limited to, ensuring that
rope descent systems
and the suspension devices for the rope descent systems are installed and
used in accordance
with ANSI/IWCA I-14.1.CA
