Nation Standards Institute (ANSI)/American Society of Mechanical Engineers (ASME) 1996 consensus standard A17.1 - Safety Code for Elevators and Escalators): Retail Office Building - On October 20, 2004 an employee sustained severe crushing injuries to an arm and leg from the material handling device becoming detached from its' connection point. In addition to this deficient attachment point, the material handling device was inadequately installed and constructed and did not have appropriate safety features and signage in accordance with the ANSI/ASME Safety Code enforced at that period in time - ANSI/ASME A17.1 - 1996. Based upon the approximate date of installation and the size of the car used to transport material between the fourth and basement floors, this device was classified as a freight elevator (reference ANSI/ASME A17.1 - 1996). Some design, construction, installation, and safety features for these devices include, but are not limited to: a)Appropriate construction and fire resistance rating of materials used to construct hoistway enclosure b)Appropriate vertical or horizontal sliding doors of solid fire rated construction c)Safety interlocks on doors d)Appropriate safety gate on car e)Overlimit (travel) protection switches f)Overlimit speed protection switches g)Travel rails for car constructed of steel or other material (not wood) h)Car constructed of steel or other material (not wood) i)"Call" and "Send" buttons for activating travel of car j)Appropriate warning signs (i.e., indicating "No Riders") k)Data plates indicating name of lift manufacturer, travel speed, weight capacities, etc. ABATEMENT NOTE: Should a decision be made by the company to continue to utilize such a material handling device, the device shall be installed to meet the applicable ANSI/ASME requirements for the latest edition of these standards (2000). The device installed must meet the safety standard requirements for a freight elevator, dumbwaiter, or material lift. The device shall be installed by a recognized and experienced business who specializes in such devices as described above. Additionally, such device chosen shall be inspected by the State of Maine Board of Elevator and Tramway Safety at established intervals.d
