Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not provide a place of employment that was free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard while lifting a casing sleeve with a drill rig. 696 W. North Avenue jobsite - Employees exposed to a crushing/struck-by hazard while lifting a casing sleeve from a caisson hole with a drill rig. The sling used in the rigging was not securely hitched to the casing sleeve and did not provide control of the load. Among feasible means of abatement would be to comply with The American Society of Mechanical Engineers, ASME B30.92003 Standard for Slings, and; a)Ensure that slings are hitched in a manner that provides control of the load. b)Prohibit employees from standing under or near a suspended load. NO CERTIFICATION NOR DOCUMENTATION REQUIRED FOR THIS ITEM.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not provide a place of employment that was free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards while using a drill rig service winch with a deficient wire rope end termination. 696 W. North Avenue jobsite - The U-bolt wire rope clip end termination for the wire rope on the drill rig service winch did not have an adequate amount of turn back of wire rope. Among feasible means of abatement would be to comply with the Wire Rope Technical Board's "Wire Rope Users Manual" and the SoilMec R-312/200 Hydraulic Drilling Rig Operation and Maintenance Manual regarding specifications for making wire rope end terminations. NO CERTIFICATION NOR DOCUMENTATION REQUIRED FOR THIS ITEM.
