Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment or a place of employment which were free of recognized hazards
that likely to cause
serious physical harm or death in that the employees were exposed to hoist
collapse or falling
equipment.
Among Feasible Abatement Method is to comply with the ANSI Standards for
Underhung Hoist
ANSI B30.16-1993, such as but not limited to the following:
1.  Provide frequent and periodic inspections of hoist and its accessories.
2.  Qualify the I-beam, the load bearing part of the hoist and other
linkages.  Mark the beam
with the rated load capacity.
At the Shop - A heavy Capacity Test Truck was used to test weight scales
at customer
location.  The hoist installed inside the truck was lifting approximately
2000 lbs load at any
given time.  The total lifted weights were 20,000 lbs.
The hoist was not inspected regularly and frequently.  The I-beam was not
qualified
nor marked
with the rated load capacity.
Abatement certification is required for this item in accordance with the
requirement of 29 CFR
1903.19(c).

being struck by a falling load lifted by the custom made lifting device:
Custom made spreader bar was used at the customer's location to lift sets
of weights from the
truck to the weight scale and back to the truck.
Feasible and acceptable abatement methods include, but are not necessarily
limited to:
Comply with ANSI B30.20 - Below the Hook Lifting Devices and:
1.   Provide the rated load on the custom made lifting device:
2.   Design the lifting device to withstand the forces imposed by its
rated load with a minimum
design factor of 3.
3.   Provide initial, frequent, and periodic inspections of the lifting
devices.
4.   Ensure that initial proof testing of 125of the rated load was
conducted.
A heavy Capacity Test Truck was used to test weighing scales at the
customer locations.
The hoist was installed inside the truck to unload and load 4 - 500 lbs
weights using a spreader
bar as a lifting device.  The bar was not qualified nor marked with the
rated load capacity. One
of the hooks attached to the bar had a broken tip.
Abatement certification is required for this item in accordance with the
requirement of
29 CFR 1903.19(c).
