Section 6(a) of the Puerto Rico Occupational Safety and Health Act:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or physical harm to employees
in that employees
were exposed to:
Seccion 6(a) de la Ley de Seguridad y Salud en el Trabajo de Puerto Rico:
El patrono no
proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten
causando o que
puedan causar muerte o dano fisico a los empleados, por cuanto empleados
estaban expuestos
a:
a)Stroke and/or crush by the automatic glass handler of the Maver machine
in
the glassware area.  The glass handler perimeter was guarded with a fence
barrier having an entrance gate protected with an electronic device,
interconnected with the machinery control, so that in the event the gate be
opened for any reason, the cycle of the machine be interrupted.  At the
time of
the inspection employees were operating the machine with the door open. The
employee protection system was inoperable.
Among other methods one available and feasible method to correct this
hazard
is to replace the sensing device and install it such a way that a
malfunction of
the device rendered the machine inoperable.
a)A ser golpeados y/o aplastado por la acarreadora automatica de laminas de
cristal de la maquina Mave en el area de cristaleria.  El perimetro de la
manejadora de cristal estaba protegida por una verja que tenia un porton de
entrada protegido por un dispositivo de proteccion electronico
interconectado
al control del equipo, de forma tal que en la eventualidad de que el porton
fuera abierto por alguna razon, el ciclo de la maquina era interrumpido. Al
momento de la inspeccion los empleados estaban operando la maquina con el
porton abierto.  El sistema de proteccion para los empleados estaba
inoperante.
Entre otros metodos un metodo disponible y apropiado para corregir este
riesgo es reemplazar el censor e instalarlo de forma tal que cualquier
malfuncionamiento del dispositivo haga la maquina inoperable."
