Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees,
while engaged in an air test of ductile iron pipe, were exposed to being
struck by a ruptured or
dislodged pipe plug:
a) Wastewater lagoon construction site, Buckingham Township, PA - On or
about June
10, 2005 employees engaged in an air test of a 36 inch internal diameter
ductile iron pipe
were exposed to injury from being struck by a ruptured or dislodged Lansas
Domehead
pipe plug.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to utilize the recommendations of the manufacture contained in the Lansas
Products publication
entitled Safety Instructions for the Proper Use of Pneumatic Pipe Plugs.
These
recommendations
would include but not be limited to the following:
1. Utilize a properly calibrated gauge for the continuous monitoring of
inflation pressures
and back test pressures.
2. Use extension hoses and poly lift rope/inflation lines.  Be sure you
are using a length
that allows you to stay clear of the "DANGER ZONE" while pneumatic plugs
are in use.
3. Always stay out of the "DANGER ZONE".
4. Blocking or bracing must be used to prevent the movement or complete
dislodging of
pipe plugs.
Abatement certification required within 10 days after abatement date.  The
certification shall
include a statement that abatement is complete, date and method of
abatement, and states
employees and their representatives were informed of this abatement.tives
