Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard of up to 18 feet in height while positioned on an inadequately guarded work platform on an operator-up high lift truck. a) Production Area: On or about 12/2/05: Employee(s) were not provided with an appropriate safety platform to access upper platform adjacent to an office in that: Employee(s) were standing on a grated shop made platform with four metal post at each end with a chain across the ground entrance side and a top rail at each end of the platform. The grated platform was resting on the forks of the Crown 234 forklift raised to a height of approximately 18 ft. exposing them to a fall to the concrete floor below. Among other methods, one feasible and acceptable abatement method to correct this hazard is to comply with Industrial Truck Standards Development Foundation standard ITSDF B-56.1 - 2005, Safety Standard for Low Lift and High Lift Trucks, thus ensuring that proper equipment, procedures, and training are provided and utilized for elevating personnel on powered industrial trucks. for example, if a work platform is used to elevate personnel on a truck designated and intended for handling materials, the requirements of sections 4.17 and 7.37, which specifically address elevating personnel and platform design, are applicable.y
