IC 22-8-1.1 Section 2: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Job site ------ On August 9, 2005 the employer did not prevent excessive lateral bending of trusses during handling and installation. Among other methods, one feasible and acceptable abatement method to correct this hazard is to use the spreader bar as a stiff back.

IC 22-8-1.1 Section 2: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Job site ------ On August 9, 2005 the employee working on the top plate of the west wall, 13 feet 6 inches above the lower level, without fall protection, was exposed to unprotected threaded anchor bolts on the lower level which were not guarded or covered in any manner to eliminate the hazard of impalement. Among other methods, one feasible and acceptable abatement method to correct this hazard is to fasten a bottom plate over the top of the anchor bolts with the appropriate size washer and threaded nut to cover the anchor bolts or to put protective caps over the bolts.
